Summary

A systematic review of 24 studies (2020–2025) found that genotype-guided warfarin dosing nearly quintupled time in therapeutic INR range versus standard care, and decision models supported cost-effectiveness of genotype-guided antiplatelet therapy. However, African-ancestry individuals comprised less than 1% of global PGx participants, and evidence certainty ranged from very low to moderate, with barriers including high testing costs, workforce shortages, and low physician genetic knowledge.
